Long Term Condition (LTC) Services

Here at Cromwell Pharmacy we offer LTC (Long Term Condition) services for patients who take multiple medicines for more than one chronic health condition.  These services are designed for people with long-term illnesses who may need support with their medicines. 

LTC services might include:

• reconciliation of prescriptions, to ensure your medication is always up to date;
• synchronisation of prescriptions to keep them aligned;
• the disposal of unused or expired medicines;
• regular proactive contact with you or your carers;
• medicine management solutions such as compliance packaging of medicine - we use Medicopacks; and
• providing a medication card every time your prescription changes.
